How would a (vegetarian) fish get close enough to do any damage?
Especially one so large it weighs 50lbs, it would get kicked and tangled in the swimmers legs before it got anywhere near.
South Americans are not likely to swim in rivers anyway, and certainly not naked, there are too many parasites.
