I heard the interview on the radio this morning by the present head of the CQC. It would seem that they are still hiding the names of those responsible. This is yet another result of the cult of pretending that a person trained in "management" can do any job even if they know nothing about the job. All these protect each other , sometimes criminally, to protect their lifestyle.
